Abstract

This study evaluated the record management practices in the Admission Office of a Novaliches District Hospital to serve as a basis for system enhancement. It examined how patient records are created, stored, retrieved, and protected to ensure accuracy, organization, and security, while identifying areas for improvement to enhance operational efficiency. The study used a descriptive-quantitative research design. A structured survey questionnaire was given to twenty (20) admission office staff who were directly involved in handling patient records. Purposive sampling was used to select the respondents. Data were analyzed using frequency, percentage, mean, and other statistical tools to describe the current record management practices, system efficiency, and challenges encountered by the staff. The results indicated that respondents strongly agreed that current record management practices are accurate, organized, timely, compliant, and efficient in processing, integration, digital utilization, and data security. However, challenges such as limited staff training, incomplete patient information, system downtime, and weak internet connectivity were identified, and a significant relationship was found between record management practices and system efficiency. The study shows that effective record management practices improve the efficiency of hospital admission operations by ensuring accurate, accessible, and secure patient records. Although current practices are generally effective, continuous staff training, system upgrades, and digital improvements are necessary to sustain and further enhance performance.
